Please call our main office at 502-423-9595 to schedule an appointment at either of our locations. When emailing please include your day time phone number so we can reach you during normal business hours.<br><br> Our telephones are answered by our staff at our main office Monday  Friday from 8:00 am to 4:00 pm. Our physicians are available for emergent needs when the office is closed.<br><br> We schedule patients in our main office from 9:00-4:00 Monday  Thursday and 9:00 am-11:30 am on Friday.<br><br> Dr. Reed Nett & Dr. Joilynn Evans see patients at our satellite office from 9:00 am -4:00 pm on alternating Tuesdays. <br><br> Our main office is located at 4801 Olympia Park Plaza, Ste. 2200 which is located right behind The Summit Shopping Center. Directions to our main office are: <br> <ul> <li> Take exit 34 from the Gene Snyder Expressway (Crestwood exit) towards the Summit </li> <li> Turn right onto Springdale by Barnes & Noble <li> Our building is a 4 story building on the right  approximately ½ mile <li> Please do not be confused with the two story office condos also named Olympia Park. </ul> Our satellite office is located in medical center building at 1905 West Hebron Lane, which is attached to Jewish Hospital Medical Center South. We are located on the 2nd floor in suite 201. When you exit the elevator on the 2nd floor simply take a left and follow the hall into our waiting area. <br><br> Please remember to bring your most recent insurance card and photo ID.<br><br> At the bottom of this page you will find a <b>new patient form</b> and <b>medical history form</b>. These need to be downloaded, completed and returned to us via fax, email or mailed at least one week before your appointment.
